Insurance
Our office files claims as a courtesy to our patients; and in doing so we accept assignment of employee benefits. Claims will be filed on your behalf after all deductibles and co-payments are made. Patients are responsible for providing documentation of employee benefits prior to each appointment.

We will wait a maximum of 45 days for filed claims to be paid by your benefit company. Since we are not part of the contract with your employer and your benefit company, their greater obligation is to you, the patient. Therefore, if your claim has not been received within 45 days, we will contact you and ask for your assistance in working with your benefit company.

All account balances older than 45 days will be paid by the patient. Should the benefit company pay the claim at a later date, the amount of the claim will be forwarded to you immediately.

Due to recent changes in insurance, we are no longer able to file claims for secondary benefits. However, we will gladly provide the information and documentation necessary for you to file your claim.
